110-300-0210(1)(2) During a health and safety inspection, the licensor reviewed child files and observed some children still do not have a completed CIS form or a department approved certificate of exemption form. 110-300-0210(3) During a health and safety inspection, the licensor reviewed child files and observed some children still do not have a signed and dated statement detailing when the child’s immunizations will be brought up to date and stating they understand their child will be excluded from care if the immunizations are not completed within 30 days of the specified due date.
